Always confirm with your health insurance plan before receiving care at any Cambridge Health Alliance location to ensure you understand your covered benefits and costs.
The plan you have chosen with your insurance company will determine the level of coverage available to you. Some insurances have different levels of costs depending on where you receive your care and what type of services are provided. In the event your insurance plan is listed below, please be aware that your benefits and costs may vary.
Depending on your plan, referrals or authorizations may be required. Not all services may be covered by your insurance, so it is best to review with your insurance plan to confirm.
Many plans use other insurance companies to cover services such as Behavioral Health, Dental or Eye services. Please contact your health insurance company to review what is covered under your plan.
CHA does accept other Commercial insurance plans. If your health insurance plan is not listed below, please contact them to confirm what coverage may be available to you.
If you are a member of one of the below Affordable Care Organizations, you will not be covered for a Primary Care Visit. These plans are only accepted for specialty services:
