    Don't have a MyCHArt account?

    • Complete a Medical Record Release Form and fax or mail it to us

      • Email us a scan of your hand-signed form as a PDF file

    There may be a cost for copies:

    If you want copies, we will charge a reasonable fee for them. We will also charge you for postage costs, if you want us to mail the copies to you.

    Use our Authorization to Communicate and Obtain PHI

    This lets CHA communicate with or obtain medical records from other providers and healthcare organizations.

    You can print these forms and fax them, mail them to us.
