CHA uses a state-of-the-art electronic medical record system. This is a safe and efficient way for us to track your health and give you better care.

Transfer your medical records to CHA

Use our Authorization to Communicate and Obtain PHI form. This lets CHA communicate with or obtain medical records from other providers and healthcare organizations.

You can print these forms and fax them, mail them to us or drop off in person.

There may be a cost for copies

To access your records, you must sign a request form that you can get from our HIM/Medical Records Department (see above). If you want copies, we will charge a reasonable fee for them. We will also charge you for our postage costs, if you want us to mail the copies to you.
