Medical Records

Medical records keep track of your medical history and care.

In the past, medical records were written on paper and kept in folders. Today, we use a state-of-the-art electronic medical record system. This is a safe and more efficient way for us to track your health and give you better care.

Do you need your medical records from CHA?

You can get copies of your records by completing the Medical Record Release form

Transfer your medical records to CHA

The Authorization to Communicate and Obtain PHI form lets CHA communicate with or obtain medical records from other providers/healthcare organizations.

You can print these forms and fax them, mail them to us or drop off in person.
