Our Philosophy and Mission

Older adults often need extra support to remain independent in their homes and communities. That’s why we are dedicated to serving their needs.

Our mission is to provide a comprehensive system of care that maintains the health, dignity, and independence of our members. Our top priority is to provide excellent support and care to members.

Older adults and their families will find a team who will work with you in a partnership of caring. All our work is done in a manner that maintains the dignity of our members and is sensitive to their preferences.

More about PACE

The Elder Service Plan of CHA was established in 1995 to provide a new option for older adults. Over the past 20 years, we have provided support for thousands of Massachusetts adults and their families, and we would be proud to provide our services to you.

We are part of the national PACE Program, Program of All-inclusive Care for the Elderly. These programs offer coordinated, interdisciplinary care for people 55+ who have medical needs but want to continue to live in the community for as long as possible.
