Each member gets a customized plan to meet his or her needs. This includes all the care and services covered by Medicare and Medicaid as well as additional services authorized by the care team. The goal is to provide the services members need in the right location, at the right time.
If you are a member, your plan may include medical care, meals, social activities, physical therapy, home care, and more. Services are provided in homes, in the community, and at our care center.
Specialty care is available to meet our members’ needs. We work with a network of specialists, including the outstanding medical team at Cambridge Health Alliance and our partner institutions.
Depending on your needs, the plan of care may include:

As an Elder Service Plan member, your Part D-covered drugs and other necessary medication are provided through the program. You don't need to join a separate Medicare Prescription Drug Plan.
Participants agree to receive all covered services through the Elder Service Plan of CHA and our contracted network of providers. Any non-emergency care or services must be approved by the Elder Service Plan of CHA in order to be covered. All approved services are provided with no co-payments or out-of-pocket expenses.
Please note that participants may be fully and personally liable for the costs of any unauthorized non-emergency care or services provided outside the Elder Service Plan.
