What are Group Visits?

Group Medical Visits are shared medical appointments, where patients with similar medical conditions have an appointment as a group. Each patient’s participation is strictly voluntary. You decide what you wish to share with the group or if you would prefer to share privately with the provider team. Research shows these visits can help you understand your health and take steps to feeling better. When you join a CHA group, you get health care and education in a supportive setting.

Our Wellness group model is designed to deliver group-based care for management of chronic diseases. Groups are facilitated in multiple languages and supported by a variety of health professionals. We offer groups at clinical sites and online. 

Cambridge Health Alliance Group Visits Program aims to aid in the improvement of health-related quality of life, patient satisfaction, patient trust in their clinician, coordination of care, patient efficacy in managing their health and more culturally competent care around chronic conditions.

Note: We are always accepting referrals for new group members. If our current groups are full, you may be placed on a waiting list.

  • Group Prenatal Education & Care w/ CHA

    Prenatal Care at CHA

    Offered via Telehealth using Google Meet

    This group aims to inform you about your prenatal care with CHA. We hold space for any questions, offer guidance around your pregnancy, care, and delivery.  After speaking with our nursing team you will be sent a Mychart message with a meeting link to join us at a later date.

    We recommend that you come to the visit because you will:

    1. Get the opportunity to connect with other pregnant people in a safe setting and learn from and support one another.
    2. Have more time to talk about the culture, FAQs, and preparing for childbirth at CHA
    3. Speak one-on-one with a doctor during the appointment to talk about your personal concerns and pregnancy.

  • Acupuncture

    Generally called Community style acupuncture. Patients will have their vitals taken and complete a check-in held in an intimate group setting of about 4-8 patients. Acupuncture services will be completed for group participants.

    These groups run in a 6-7 week series. Patients must commit to attending a full series.

  • Healthy Homes Healthy Families

    This group program will focus on feeling good about yourself, how to build healthy habits and fun ways to get your body moving for the whole family! Through this group, we will incorporate practical ways to eat healthy both at home and when you are out. We will also share tasty recipes that practice these new healthy eating habits. Some sessions will even be outside to explore enjoyable ways to incorporate physical activity into your daily routine!

    For children 5-10 years old and their parents or guardians. 

    This program is 8 sessions and will be held at Malden Care Center, Thursday afternoons from 4-5pm, biweekly.

  • Diabetes Education

    The Diabetes Education class is now accepting new members! This one-hour class meets virtually as a 2 class series and aims to provide education in a structured, supportive group environment to discuss living with diabetes. This group will empower members to make lifestyle changes to improve their health and give the basic information to participate in their medical decision making. Members will be educated on medications used to treat diabetes and beneficial lifestyle changes through diet and exercise. 

    For: English speaking patients with a diganosis of diabetes

  • Hypertension Education

    The Hypertension Education class is now accepting new members! This one-hour class meets virtually and aims to provide education in a structured, supportive group environment to discuss living with hypertension. This group will empower members to make lifestyle changes to improve their health and give the basic information to participate in their medical decision making. Members will be educated on medications used to treat hypertension and beneficial lifestyle changes through diet and exercise. 

    For: English speaking patients with a diganosis of hypertension

  • Living Well A Wellness Group

    Meet with our healthcare provider, their team, and other patients living with similar chronic disease, chronic pain, sleep problems, fatigue, and others in a shared medical setting/group.

    Learn ways to care for yourself you may not have thought of before. Have the chance to learn new life skills like stress reduction, gentle movement, and  healthy cooking, mindfulness and pain reducing techniques.

    For: Adults 18+

  • Healthy Living - A Wellness Group for Diabetes Management

    Get your Diabetes care in a Group! Increase your knowledge of diabetes, set goals, and share strategies with fellow patients. Create a safe and understanding community with peers to discuss struggles, victories, and the journey of managing your health with Diabetes.

    You will have the opportunity to meet with your provider team and adjust your medications as needed.

    For: Adults 18+ English speaking

  • Life and Recovery

    Struggling with opioids? Looking for a new chance at recovery?

    Come join our group visits where you get expert care in a supportive setting. We are a low level care group focused on addiction recovery and life skills.

  • Mindfulness Training for Primary Care (MTPC)

    MTPC is a well-tested clinical treatment program proven to help participants with depression, anxiety, chronic pain, and different kinds of stress. The group provides:

    • A safe, motivating environment to help you integrate mindfulness into your daily life. 
    • A chance to explore your Core Values and to support you in living a more satisfying life. 
    • An opportunity to learn skills to help you tackle health-related challenges.

    Duration: 8 weekly sessions, 2 hours each

  • Group Sante - A Haitian Creole speaking Wellness Group

    Klike la a wè detay sou gwoup la an kreyòl ayisyen

    This group aims to manage chronic disease for our haitian creole speaking community. This group is home to a great community of peers using movement, knowledge, and support to help each other gain better control and understanding of their medical conditions. We partner with Nutrition, Physical Therapy and other departments to bring you high quality care. There is peer discussion, education, exercise and many other wellness activities and demos. 

    For: Adults 18+ Haitian Creole speaking

  • Being Healthy Together - A Wellness Group for Children

    Whole body care is at the core of this group's mission. We work with young patients to help make sustainable changes in lifestyle, behavior and mindset, mindfulness, body image, movement and more. Group sessions include a teaching element, a sharing element, fun activity breaks, and some goal setting.

    Treating and preventing childhood obesity helps protect your child's health now and in the future. It requires children (and their families) to show personal accountability, motivation to change daily habits and continuing lifestyle changes after group.

    For: CHA patients under 18 years of age

  • Healthy Together - A Wellness and Healthy-weight Group

    Healthy Together - A Wellness and Healthy-weight Group is a group program for adults 18 and older who are interested in getting guidance and education on overall wellness and healthy weight. Our goal is to incorporate wellness into your lifestyle to prevent and reduce symptoms from chronic illness focusing on overall health and weight management. This is an evidence-based approach to learn healthy and mindful eating, goal setting, tracking, stress management, sleep hygiene, physical activity, meal plans and prep, healthy recipes and food demonstrations with individualized medical management.

    For: English speaking Adults 18+

  • Balanced Beings - A Wellness Group

    Looking for a little extra support to achieve your health goals?

    This group runs for 8 week sessions. Each session includes 20 minutes of mindfulness based movement, 20 minutes of education on healthy living, and 20 minutes of open conversation and questions. The goal of the group is to establish a community for sustainable health changes. 

    Together, we will explore nutrient dense nutrition, joyful movement, sleep optimization, and incorporating mindfulness into everyday life!

  • Health For Every Body - A Wellness Group for Weight Inclusive Care

    Are you sick of dieting? Are you tired of trying to lose weight? What if we said you can be healthy without changing your body? Join us at Wellness for Every Body to join a community where we practice evidence based techniques to be healthy without focusing on the number on the scale.

    Weight-inclusive, whole body care is at the core of this group's mission. Our goal is to bring a weight neutral approach to health to our patients by focusing on lifestyle behaviors, intuitive eating, movement, mindfulness, and body image. This group visit is based on the Empowerment-based model for patients to actively participate in their health and in their group community, with a structure that focuses on nutrition/intuitive eating, movement, and mindfulness.

    For: English and Spanish speaking CHA patients, 18+
