When you move to a nursing home or need a short-term stay after a hospital visit, you can still use CHA for your care. Providers from the CHA Nursing Home Program see patients in select Skilled Nursing Facilities, providing expert care in the most coordinated way.

A partnership that works for you

CHA partners with several local nursing homes to see patients. This work includes:

  1. Providing primary care in the nursing home.
  2. Collaborating with nursing home staff to develop care plans and meet ongoing health needs.
  3. Helping patients who go from the hospital to the nursing home and then back home.

This program lets us give each patient the right care in the right setting to address his/her health needs. This collaboration and coordination of care can keep you healthy and/or speed your recovery.
