CHA Primary Care: Transforming into Patient-Centered Medical Homes

PCMH PRIME CertificationThe Patient-Centered Medical Home (PCMH) is a new way to provide primary care services.

As of December 2016, all 12 CHA primary care practices have been designated as Level III Patient-Centered Medical Homes by the National Committee for Quality Assurance (NCQA) and have received the MA Health Policy Commission's Patient Centered Medical Home PRIME Certification for behavioral health integration.

What does this mean?

When you choose a Patient-Centered Medical Home, you receive better, more personal care. Your CHA doctor leads a team of nurses, medical assistants, front desk staff, consulting psychiatrists, integrated therapists, care managers, pharmacists, health educators and care coordinators. This team will work together to support your needs, monitor your health over time and coordinate your care.

Your care team will help you stay healthy by encouraging you to:

  • Setting your own health goals
  • Getting to know your care team and help them get to know you
  • Making your best effort to come to each appointment (or call if you need to reschedule)
  • Sharing your medical history and records from every place you get care
  • Bringing your insurance card and medications to every visit
  • Signing up and using the MyCHArt patient portal
  • Discussing your specialty care needs so we can help coordinate your care

How do I know where my clinic is in this transformation?

All CHA practices are now certified as Patient-Centered Medical Homes and have the NCQA seal and HPC PRIME seal on their web pages.

How can I be more involved in this change?

Each CHA Care Center has a Practice Improvement Team (known as “PIT team”), with both staff and patients. We are always looking for more patients to join. If you are interested in being part of your clinic’s PIT team, please speak with your doctor.
