CHA's Elder Service Plan Moves to East Cambridge

CHA East Cambridge Care CenterCAMBRIDGE, Mass. – Cambridge Health Alliance (CHA), an academic community health system serving Cambridge, Somerville and Boston’s metro-north region, has moved its Elder Service Plan program to 163 Gore Street in Cambridge next door to the CHA East Cambridge Care Center.

The Elder Service Plan relocation gives the more than 300 members in the program access to a new facility where they can socialize and receive care. Some of the features of the new space include more than double the number of primary care exam rooms, seating for about 80 participants in the day center (up from 40 in the prior space), a large rehab gym, and three private rooms for social work meetings with participants.

The Elder Service Plan of CHA was established in 1995 and, as part of the national PACE Program (Program of All-inclusive Care for the Elderly), provides coordinated, interdisciplinary care for people 55+ who have medical needs but want to continue to live in the community for as long as possible. The PACE program is an alternative to nursing home care, allowing frail older adults to live in their homes and communities with proper support. Over the past 20 years, the program has helped thousands of local adults maintain their health, dignity and independence.

Each program member gets a customized plan to meet his or her needs. This includes all the care and services covered by Medicare and Medicaid as well as additional services authorized by the care team. The goal is to provide the services members need in the right location, at the right time. In order to qualify for the Elder Service Plan adults ages 55+ must live within the PACE program service area at the time of enrollment. The Elder Service Plan covers Allston, Arlington, Brighton, Cambridge, Chelsea, Charlestown, Everett, Malden, Medford, Somerville and Watertown.
