Some insurances have different levels of costs depending on where you receive care and what services are provided. Please be aware that your benefits and costs may vary.

Your health insurance plan may require referrals or authorizations for certain services.
Many health plans use other insurance companies to cover services such as Behavioral Health, Dental, or Eye services. Please contact your health insurance company to review what is covered under your plan.
Health insurance contracts can change and the list below may not have all plans currently accepted at Cambridge Health Alliance for primary care. If your health insurance plan is not listed below, please contact them to ask what services are covered for you at CHA.
If you have a MassHealth ACO plan, you will only be covered for Primary Care if you have Tufts Health Together with CHA.
