Making
an Appointment
In most cases, you must have approval from your primary
care provider before making an appointment with a surgeon. If you
have a question about the process, please contact your insurance
company or primary care provider directly.
If you want to be seen by a CHA surgeon, please ask your primary
care provider for a referral. Regardless of where you go for surgery,
you should feel comfortable with your care; all of our surgeons
give second opinions. If you have any questions about the referral,
please call us at 617-665-2555.
What to Bring For
Your Appointment
Please plan to arrive 15 minutes early for your first visit to
our surgeons. This will give you time to register and fill out any
necessary paperwork.
Please also bring the following with you if possible:
- Any X-rays or other relevant diagnostic tests
- Your medical record that relates to your visit
- Current medications and dosages that you are taking
- Name and phone number of your PCP
- Health Insurance information
Co-payments not covered by insurance are expected at the time of
service.
If you need to cancel your appointment for any reason, please call
us at 617-665-2555 as soon as possible so we can reschedule.
