The CHA Malden Family Medicine Center is a
state-of-the-art primary care facility that opened in June 2007
at 195 Canal Street in Malden, MA - right next to Anthony's restaurant.
More than 30 Family Medicine doctors work on site

here for a list). These doctors see patients of all ages.
The new site was designed as a model practice that
would use the finest technology to benefit our patients. For example,
all our doctors use hand-held computers and a new electronic medical
record to improve both the quality and delivery of your care.
We also have lots of FREE parking.

Do you know about MyChart? MyChart is CHA Malden Family
Health Center's new electronic medical record system which
allows patients to access their medical records, check on
upcoming appointments, send secure messages to their provider,
and more. Parents of pediatric patients can also register
a MyChart page for their child. MyChart is a convenient way
to access information about your healthcare right from your
computer at home.
If you would like to learn more about MyChart, or would like
to request access to MyChart, simply ask about it at your
next visit to your primary care provider. They will give you
information about terms of use and privacy.
