CHA House Calls Program

This service provides primary care to home-bound older adults who wish to remain at home and do not have access to the doctor's office. Through the House Calls program, doctors and nurse practitioners visit patients' homes and coordinate all social and health care with appropriate community partners. Currently House Calls is available in Cambridge and Somerville.
