Continuity Clinics

Each trainee is assigned to one of our three continuity clinic training sites where he/she will develop an outpatient practice during the three years of training: the Primary Care Center, Somerville Hospital Primary Care, or Windsor Street Care Center. Each of these sites are actively engaged in Patient-Centered Medical Home (PCMH) transformation, and residents in our program are uniquely involved in all aspects of this transformation. We are delighted that each of the ambulatory training sites for our Internal Medicine Residency Program have achieved Level 3 PCMH recognition from the National Committee for Quality Assurance (NCQA).

At all continuity sites, the resident physician acts as the primary care physician for his/her panel of patients, with close preceptor supervision that is tailored to individual learning needs. Residents gain comprehensive experience in chronic disease management, acute care office visits, and screening and prevention across the full spectrum of adult age groups.
