SOMERVILLE, Mass. – Cambridge Health Alliance (CHA), an academic community health system serving Cambridge, Somerville and Boston's metro-north communities, will open its 13th primary care center, CHA Primary Care Assembly Square, in Somerville on June 4. The site is located at 5 Middlesex Avenue.

In 2017, roughly 22,000 Somerville residents visited a CHA primary care practice at least once. Primary care at CHA is recognized for excellence and delivering for high quality and compassionate patient care. Twelve of its centers have been recognized by The National Committee for Quality Assurance (NCQA) as Level 3 Medical Homes - one of the highest standards of patient-centered care. NCQA is a private, nonprofit organization known as the industry's gold standard for health care quality improvement. These 12 centers have also received Patient-Centered Medical Home PRIME certification from the Massachusetts Health Policy Commission (HPC). Recognition, from NCQA and the HPC, highlight CHA's leadership in primary care by combining access, teamwork and technology.

Starting in May, patients of all ages can schedule a new primary care visit with one of CHA's expert family medicine doctors. The medical team is bilingual in English, Spanish, Portuguese and Russian. To schedule an appointment call 617-665-1305 or visit www.challiance.org/assemblysquare.

In addition to Primary Care, services on-site will include Women's Health, Family Planning, Mental Health, Laboratory, Nutrition Counseling, Social Services and coordination of specialty care. The site will also offer walk-in care for CHA patients. "As the population of Somerville continues to grow and change, so do we. Our goal is to meet the health needs of all members of the community while we work to keep people well -- all in a setting where patients and staff feel valued and respected," said Mary Cassesso, president of the CHA Foundation and CHA's chief community officer, also a lifelong resident of Somerville.

The front door of the practice is located on the left side of the building, facing Home Depot. There is free parking and the building is accessible by public transportation on the Orange Line or by bus. Hours of operation will be Monday and Tuesday, 8 a.m. - 8 p.m., and Wednesdays, Thursdays and Fridays from 8 a.m. – 5 p.m. Evening hours will be offered on Wednesdays and Thursdays in September.
