ELDER SERVICE PLAN OF CAMBRIDGE HEALTH ALLIANCE

The Elder Service Plan is a comprehensive program designed for frail elders who are nursing home eligible but wish to remain at home.

The program provides general primary care, rehab, and wellness services at an Adult Day Center and at home. It also uses a team approach to coordinate all health, medical and social services needed to maintain the frail elder's independence.

It is currently available in Somerville, Cambridge, Malden, Allston, Arlington, Brighton, Chelsea, Everett, and Medford.

The Elder Service Plan of Cambridge Health Alliance operates under a contract with the Massachusetts Departments of Medical Assistance and the Centers of Medicare and Medicaid Services.

An individualized care plan may include:

• Family Caregiver Support
• Personal Care and Homemaking
• ESP Day Health Center
• In-Home Services
• Transportation and Meals
• Primary Medical Care
• Physical, Occupational and Recreational Therapies
• Specialty care including dental, podiatry, optometry, and audiology
• Prescription drugs, medical supplies and equipment
• Emergency Care
• Hospital Care
• Nursing Home Care
