PSYCHIATRY TRANSITION SERVICE
CHA has a new Transition Service at the Cambridge
Hospital campus that can help our patients while they change
providers or move to different levels of care.
Patients who visit us will find a team of social
workers, nurses, and doctors who can help you on a short-term basis.
Our services may include case management, medication management,
and/or group therapy, based on your specific needs.
Who is the service for?
The service is suited for CHA patients who are in-between
providers, or in-between levels of service. This includes patients
who are transitioning from inpatient to outpatient care.
How do you get involved?
Just ask your CHA mental health provider if the
Transition Program is right for you.
