CHA Clinics Named National Best Practice Sites
Recognition for Medical Home Transition Comes After Rigorous Review
Cambridge, MA…Cambridge Health Alliance (CHA) announced
today that two of its family health practices in Revere and Somerville
have been selected as national best practice sites for team development
by the Commonwealth Fund (Qualis) Safety Net Medical Home transformation
initiative.
The Commonwealth Fund, a private foundation that aims to promote
high performing health care systems that achieve better access,
improved quality, and greater efficiency, this year launched a national
Safety Net Medical Home Initiative to provide $6 million over four
years to help 68 community health centers in five states transform
into patient-centered medical homes. Health centers in Colorado,
Idaho, Massachusetts, Oregon, and Pennsylvania will be given technical
assistance, training, and ongoing support in order to improve healthcare
delivery to patients, including better coordinated care, enhancing
access to care, improving doctor-patient interactions, and implementing
quality improvements.
"This is a great development and bodes well for our journey
toward becoming a national model as a primary care system through
our medical home transformation," said Dr. Somava Stout, medical
director of the Revere Family Health center.
The Safety Net Medical Home Initiative has scheduled a national
Webinar for Dec. 16th to share how CHA has fostered the development
of teams at multiple levels at Revere Family Health and Union Square
Family Health in its medical home transformation journey.
"This recognition acknowledges the hard work and dedication
of many and also shines a beacon of hope for our future at a very
important time," said CHA's Chief Executive Officer Dennis
Keefe.
