There are two options for submitting an application: |
| Pre-apply: complete the online application form and submit requested materials at a later time. Pre-applying will allow recommenders to submit letters of recommendation before you have submitted a final application. |
| Apply: complete the online application form and submit requested materials at the same time. |
Application Submission |
| Step One: Prepare materials for submission. |
| Before you begin the online application process, please create separate electronic files of each of the following documents. If pre-applying, you do not need to create these files before proceeding to step two. |
| All Applications: |
| Personal Statement of Goals and Interests: Please submit a 1-2 page narrative statement specifying career goals and interest in program to which you are applying. |
| Curriculum Vitae |
| Graduate Transcript: You will need to obtain an electronic image of your graduate transcript from the Ph.D. or Psy.D. program in which you are currently enrolled (“Unofficial” or student copies are acceptable. Applicants invited to interview will be asked to hand carry official transcripts in a sealed envelope) |
| Additional documents depending on the program selected. All patient identifying information must be removed from these documents before they are submitted. |
| Adult Neuropsychology Neuropsychological Test Report: You will be asked to submit 2 reports. Please prepare a separate file for each report. |
| Child/Adolescent Acute Services and Adult Psychotherapy and Psychological Assessment One Psychological Test Report and one Case Summary. Please prepare a separate file for each item. |
| All Others Case Summary: You will be asked to submit 2 Case Summaries. Please prepare a separate file for each summary. |
| Electronic File Guidelines |
| You must submit your graduate transcript as an Adobe Acrobat (.pdf), JPEG (.jpg), GIF (.gif), or TIF (.tif) file. All other materials must be provided to us in Microsoft Word (.doc), text (.txt), Rich Text Format(.rtf), or Adobe Acrobat (.pdf). Other file types will not be accepted. |
| If you have transcripts from more than one school, please prepare separate files for each transcript. |
| The file name of all documents must end with the file type that identifies the type of file being submitted. For example: sample.txt where .txt is the file type sample.doc where .doc is the file type. |
| You will be able to upload more than one file to complete each requested document. If you are using dial-up or have a slow internet connection you should split your transcript into 2 to 3 files. |
| Please note, the amount of time required to upload a file will vary depending on the speed of your internet connection and the size of the file. PDF, GIF and other image files may take longer to upload than text or Word documents. |
| Step Two: |
| You will need to complete the online application. |
| A special note about letters of recommendation |
| You will be asked to provide the names, email addresses and phone numbers of 3 individuals who will provide letters of recommendation for you. |
| The CHA online application system will contact each of your recommenders via the email address you provide and direct them to a secure link that will allow your recommender to upload a letter of recommendation on your behalf. |
| Please advise your recommenders that CHA will only accept letters of recommendation in electronic format. |
| Upload the documents you prepared for step one or indicate you are pre-applying. |
| You may only submit your application form once. Please contact us if corrections are needed. |
| Step Three: |
| We will acknowledge receipt via email when we have received your application materials and all of your letters of recommendation |

Option II: Clinical Psychology (Behavioral Medicine Clinic - one year) |

Option III: Clinical Psychology (Child and Adolescent Acute Services - one year). Fellows rotate between the Child Assessment Unit (5 months), the Adolescent Assessment Unit (5 months), and the Child and Adult Psychiatric Emergency Service (2 months). |
