CHA Senior Care Program

This program provides medical services for older adults in assisted living facilities and nursing homes near our three CHA hospital campuses.

Our staff of doctors and nurse practitioners serve as the primary care providers for older adults and we work closely with the staff at each partner facility. This practice allows us to understand and manage ongoing health needs, and make sure each patient gets the right care in the right setting for their unique health conditions.
